Stage 4 CKD clinic

I am a relatively new nurse practitioner and very new to nephrology.  The practice where I am employed is asking me to start a CKD clinic.  This clinic is to be for stage 4 patients and focused on dialysis preparation and fistula placement.  They want it to be in conjunction with the nurses that teach the TOPs class at Fresenius.  I attended my first ANNA conference in Florida and remember several discussions surrounding KDE education, however I am not able to find any information on KDE programs.  I am trying to define my role as an NP in this clinic in comparison to the role of the nurses that are already doing dialysis education.  ANY resources or advise would be greatly appreciated.
