What is everyone doing with administrating vanco during hd session? last hour? anytime?

We give all of our antibiotics after the dialysis is completed. However, some patients will terminate their treatments early in order to complete the hour-long infusion. I am not sure how much of the vancomycin is lost if given during the last hour. Do you put the machine in bypass to prevent loss of medication?

If the vancomycin has to be given during dialysis we give it in the last hour.
