alternative treatments for hypotension

A pharmasist at one of our facilities is insisting we use Mannitol or Hespan to treat hypotension instead of albumin. I thought Hespan was not recommended for dialysis pts and we haven't used Mannitol in years. Are any of you using these drugs to treat hypotension?

we have always used mannitol x2, but not in the last hour of dialysis. we also use albumin

Have you tried sodium or UF profiling to help pevent the hypotension? We have also used NS in the past to help with hypotension. If your machine has NA or UF profiling options try using one of the them to prevent adding more fluid to remove.

Please ask your pharmacist to check the 1/2 life of both mannitol and hespan - it is several DAYS, not hours. If given during dialysis, the hyperosmolal effects will continue long after the pt. is off of dialysis, i.e. it could increase intravascular volume in a person without the renal function to remove it - dangerous!
