If you provide CRRT in your facility what physician group manages the therapy....intensivist or nephrology?

Since we have two nephrologists who also work as intensivists, the CRRT is sometimes ordered by them if during the nights, etc. During the day, our nephrologists on call manage the CRRT.

Our nephrologist manages the CRRT and the ICU staff manage the CRRT therapy.

Our Nephrologists manages the CRRT in our facility, the dialysis nurses will perform the set up of the therapy and set changes every 48-72 hours, plus additional need if the patient travels for tests, clots off, etc. The ICU nurses monitor the system while it runs, change bags, etc. This causes, at times, increased instances where the dialysis nurse is called in during their off shifts to perform set ups or called for trouble shooting at all times of the day/night. It would be nice to see how other programs run their CRRT in their facility. Our medical director thinks if they give CRRT to the ICU to help manage, the Nephrologists will no longer have control of the therapy and will lose revenue. I would like to create a "business plan" to help show that if they let the ICU staff (RNs) take over the CRRT therapy, the patient will benefit because the ICU staff is there 24/7 and therapy can be initiated sooner, if the system clots off, it can be restarted sooner, etc. I also need to show the Nephrologists that they would still maintain control of the ordering of and billing for the therapy. Any help would be appreciated!
