I was taught to calculate the NSS flushes in for the total target goal at the begining of the treatment.

Recently a staff member told me at their unit they increase the goal after each 200 ml NSS flush.

I would like to know if unit are looking into different ways of calculating weight loss with heparin free dialysis. Especially on patients that are having problems with fluid removal. We have done many profiles and are still having problems with 1 patient.

I have seen many patients particularly the hospitalized ones that do not tolerate the total of NS flushes added to the UF goal at the beginning. If one is using a Crit-Line monitor you can assess if adding the flushes to the goal initially is tolerated or dangerous. If a patient doesn't have the volume to spare the goal should only be increased as the fluid is added. It also may be prudent to question whether the patient needs flushes to keep the dialyzer patent especially if the other machine indicators of clotting are not present.

I always calculate the n/s flushes into the total goal at the beginning of the treatment. I have also found that if the patient does not tolerate the ultra filtration rate very well talk to the doctor about trying different UF profiles until you find the one that best fits the patient.
