A collection of lymphatic fluid between the transplanted kidney and the bladder, originating from extravasation from the lymphatic vessels injured or interrupted during iliac vessel preparation at the time of surgical transplant. May also originate from the unligated lymphatic system from the donor kidney renal hilum. Patients may be asymptomatic with spontaneous resolution. Deterioration of renal function, inguinal edema, and ipsilateral leg edema may suggest this complication. Treatment may be achieved with aspiration for smaller lymphoceles or surgical correction may be indicated with marsupialization to allow for continued draining of the lymphatic fluid into the peritoneal cavity. Intervention is necessary for larger collections to avoid progressive loss of renal function and to restore renal function.
