One of the most common post transplant malignancies, also associated with Epstein-Barr virus (EBV) (see also Epstein-Barr virus). PTLD may present as a localized or disseminated disease post transplant. Aggressive immunosuppression increases the risk and is associated with earlier disease development. Tumors can be aggressive, progressive and potentially life threatening. Presenting symptoms may include fever, lymphadenopathy, gastrointestinal symptoms with possible obstruction, infectious mononucleosis type syndrome (may be fulminant), pulmonary symptoms, central nervous system (CNS) symptoms, weight loss, and possible decline in renal function. Initial intervention may include reduction of immunosuppression however declining renal function may result.
